Integrated Neighbourhood Teams (INTs)

What are INTs

Integrated Neighbourhoods provide proactive and integrated care to communities of 30,000-50,000. They aim to keep local people well and address poor outcomes for their local population. Integrated Neighbourhoods build on the base of primary care networks, bring all parts of the workforce together and put the people at the centre of the care they receive
